Facilitating:
  • End-of-Life planning including Advance Care Planning, Representation Agreements and other documents
  • End-of-Life (EOL) conversations and encouraging individuals to reflect and communicate ideas about values, beliefs and wishes
  • Helping individuals to make decisions for EOL, sudden illness or even critical injury

​Advocating:
  • Supporting the dying to make their end-of-life wishes a reality
  • Offering emotional, practical, and spiritual support to those living with a life-limiting illness and their companions before, during and after death.

Guiding:
  • Engaging in process of reflection to communication values, beliefs and wishes
  • Providing referrals, support networks and complimentary care options (ie: therapeutic touch, reki)
  • Demystify palliative care and range of options/choices, including MAiD, as end of life approaches


Companioning:
  • Sitting bedside to vigil
  • Being a compassionate and calming presence
  • Offering respite to family and friends
  • ​Holding space, offering companionship and engaging in active listening

I offer a free 30-minute consultation (by phone or Zoom) to discuss your needs and how I may be able to support you and your family.

Offering both in person and virtual support

Hourly Consulting Rate - $90 + GST

Packages:

ACP Planning - includes ACP and EOL discussions - 4 hours $300 + GST

Enhanced Planning - more tailored to individual needs (ACP, EOL planning, referrals)
